About the Role

Join Mount Sinai Health System as a Social Worker – Complex Discharge Team in United States.

Key Responsibilities

  • must emphasize the physical, emotional, and social needs of each patient and their care partners.
  • will work collaboratively with case management, medical teams, and leadership to follow medically and/or psychosocially high risk patients throughout adult medicine and surgery units for the primary purpose of reducing the length of stay and improving the outcomes and experience for these patients and their families who have several barriers to discharge.
  • will primarily be identified weekly, through long-stay rounds, history of multiple readmissions, referrals from unit based teams and patients known to CDT from prior admissions.
