Job Description

Location Address

4588 Paradise Blvd NW Albuquerque, NM 87114 Type of Opportunity: Full time FTE: 1.00

Summary

At Presbyterian Healthcare Services, we're creating a different kind of primary care experience for some of our most vulnerable / at risk patients. We're seeking a compassionate and experienced provider to deliver highly personalized care to a medically complex population of dual-eligible Medicare and Medicaid patients.

This unique role is designed to provide the time, attention, and resources that traditional primary care settings often cannot. You'll focus on patients with multiple chronic conditions, unmet healthcare needs and treatable medical issues that require closer follow-up and more intensive care coordination.

Job Description

As an Advanced Primary Care Provider, you will deliver comprehensive primary care services to a panel of medically complex patients who require a higher level of clinical attention and support.

Unlike traditional primary care settings, this role intentionally allows for smaller patient panels and longer appointment times, giving you the opportunity to fully assess patient needs, address barriers to care, coordinate resources, and improve overall health outcomes.

Patients in this program are those who would benefit from enhanced clinical management beyond what can typically be provided in a standard primary care practice.

Schedule: Monday through Friday

  • 8:00 AM to 5:00 PM
  • No weekends
  • No traditional high-volume clinic expectations
  • 12-14 patients per day
  • 30-60-minute appointment times

Key Responsibilities

  • Provide comprehensive primary care services for dual-eligible Medicare and Medicaid patients.
  • Manage patients with multiple chronic conditions and complex healthcare needs.
  • Diagnose, treat, and monitor acute and chronic medical conditions.
  • Develop individualized care plans focused on improving health outcomes and quality of life.
  • Address gaps in care and identify treatable or curable conditions through proactive patient management.
  • Collaborate closely with primary care physicians, care managers, specialists, social workers, and community resources.
  • Coordinate additional services and support to address medical, behavioral, and social determinants of health.
  • Educate patients and caregivers regarding disease management, treatment plans, and preventive care.
  • Foster trusted, long-term relationships with patients through compassionate, patient-centered care.

What Makes This Opportunity Different

  • Smaller daily patient volume allows for meaningful patient interactions.
  • Extended appointment times support comprehensive assessments and care planning.
  • Focus on quality and outcomes rather than high-volume visits.
  • Opportunity to work with an underserved population and make a measurable impact.
  • Collaborative, interdisciplinary approach to care.
  • Ability to connect patients with additional resources that support their overall well-being.

Additional Job Description

Required

  • Current New Mexico licensure or eligibility for licensure.
  • DEA certification or ability to obtain.

Preferred
