About the Role

Join HOSPICE OF SURRY COUNTY as a Hospice Social Worker in United States. Hospice Social Worker (BSW or MSW) Full-Time | Monday–Friday | 8:00 AM – 5:00 PM Serving the Yadkin Region Every Patient. Every Family.

Key Responsibilities

Provide emotional and psychosocial support to patients and families

Complete assessments and contribute to individualized care planning

Assist with advance care planning and end

life decision support

Requirements

Bachelor’s or Master’s degree in Social Work (BSW or MSW)

Current social work license (or eligible) in North Carolina

At least 1 year of experience in healthcare, hospice, or a related human services setting

Strong communication and relationship

Work Schedule
