Join HOSPICE OF SURRY COUNTY as a Hospice Social Worker in United States. Hospice Social Worker (BSW or MSW) Full-Time | Monday–Friday | 8:00 AM – 5:00 PM Serving the Yadkin Region Every Patient. Every Family.
Key Responsibilities
- Provide emotional and psychosocial support to patients and families
- Complete assessments and contribute to individualized care planning
- Assist with advance care planning and end
- life decision support
Requirements
- Bachelor’s or Master’s degree in Social Work (BSW or MSW)
- Current social work license (or eligible) in North Carolina
- At least 1 year of experience in healthcare, hospice, or a related human services setting
- Strong communication and relationship
