The Licensed Master Social Worker will conduct social service assessments and manage support plans to address the socioeconomic and medical barriers faced by senior members. They will also collaborate with primary care teams to provide case management and ensure accurate documentation of patient encounters.
Candidates must hold a Master's degree in Social Work and a fully licensed LMSW in the state of practice. At least one year of experience in clinical social work and strong interpersonal communication skills are required.
