About the Role

Join Legacy Health Services as a Licensed Social Worker – Long-Term Care, Ohio in United States.

Key Responsibilities

Complete psychosocial assessments, care plans, and documentation in accordance with state and federal regulations.

Provide emotional support, counseling, and crisis intervention to residents and families.

Coordinate discharge planning, including community resources, home care, and support services.

Participate in interdisciplinary team (IDT) meetings and contribute to individualized care planning.

Requirements

Required: Bachelor’s Degree in Social Work (BSW) or related human services field.

Licensed Social Worker (LSW) in the State of Ohio OR eligibility to obtain licensure.

Knowledge of long‑term care regulations and discharge planning procedures.

Strong communication, documentation, and organizational skills.
