Key Responsibilities

The Social Worker assists with the coordination of the discharge process and collaborates with the healthcare team to achieve targeted patient outcomes. They also perform patient and caregiver assessments to identify risk factors and implement interventions to ensure a successful transition post-discharge.

Requirements

Candidates must hold a Master's degree in social work or social welfare and possess MSW licensure by the South Carolina Board of Social Work Examiners. Excellent communication skills and the ability to work independently in a professional medical environment are required.
