About the Role

Join Daniel Memorial Inc as a Wraparound Coordinator (Behavioral Health) in United States. Hours: Monday to Friday: 8:00am to 5:00pm - some evening/weekend work required. Bachelor's Degree REQUIRED, and at least 1+ years' experience working or living with children / families with complex emotional needs.

Key Responsibilities

• conduct a strengths-based inventory from which an Individualized Wraparound Plan, designed to guide the deli.

• Provide care management to access and coordinate multi-agency services for the child with a serious emotiona.

• provide input as appropriate to its evolution * Participate in project evaluation and quality improvement ac.

• provide quality services to clients and families as evidenced by assessment of services and the quality impr.

• Document all care coordination activities in the Mental Health electronic record format as formulated and available.

• Utilizes various computer systems effectively to carry out assignments.

Benefits & Compensation

• Hourly rate: $21/hr.

Work Schedule

• Full-time position.

• Weekend coverage may be required.
