About the Role
The Roads Home Health Care is seeking a licensed Home Health Physical Therapist to provide skilled physical therapy and rehabilitative patient care to patients in their homes throughout South Florida. In this role, you'll evaluate patients, design individualized treatment plans, and help people regain strength, mobility, and independence after illness, injury, or surgery. Both full-time and per diem opportunities are available for the right candidate.
- Perform comprehensive in-home patient assessments and develop individualized physical therapy treatment plans
- Obtain and communicate physician orders for certification and recertification of home physical therapy services
- Deliver physical therapy modalities and hands-on treatment appropriate to each patient's condition, needs, and age
- Design home exercise programs tailored to each patient's goals and functional status
- Assess patients for durable medical equipment and adaptive equipment needs
- Evaluate and manage patient pain through appropriate physical therapy interventions
- Educate patients and family caregivers on techniques, exercises, and home safety
- Identify safety and mobility risks in the home environment and recommend interventions
- Provide clinical supervision and mentorship to physical therapist assistants
- Complete timely, accurate documentation and coordinate care with physicians and the interdisciplinary team
Job Requirements
- Current, valid Florida Physical Therapist license (or eligibility per state and CAPTE-accredited program requirements)
- Graduate of an accredited physical therapy program
- Minimum of two years of physical therapy work experience
- Current CPR/BCLS certification
- Current health certificate, physical exam, and TB clearance
- Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check
Qualifications
- Strong clinical judgment with the ability to synthesize assessment findings into an effective care plan
- Excellent verbal and written communication skills for patient education and interdisciplinary coordination
- Ability to work independently while managing a home visit schedule efficiently
- Compassionate, patient-centered approach to home health and rehabilitative care
- Strong organizational skills and attention to documentation detail
- Prior home health experience preferred
- Bilingual candidates a plus
