Key Responsibilities
Conduct comprehensive functional evaluations and develop individualized treatment plans to maximize patient independence and safety at home. Collaborate with an interdisciplinary team to deliver evidence-based interventions and ensure accurate, timely documentation in compliance with CMS standards.
Requirements
Candidates must hold a degree from an accredited Occupational Therapy program and possess a current Michigan Occupational Therapist license. At least one year of home health experience is preferred, along with reliable transportation and strong clinical skills.
