Key Responsibilities

Provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. Collaborate with the interdisciplinary team to develop and manage patient care plans while maintaining accurate and timely clinical documentation.

Requirements

Must hold a valid and unrestricted professional license in the state of practice and be a graduate of an ACOTE accredited school. Prior home health experience is preferred.
