Key Responsibilities

Provide home

based social services to patients dealing with depression, anxiety, poor nutrition or a lack of resources that make managing their injury or disease difficult

Work as part of a hospice team which may include an RN, LPN, CNA, HHA, PT, OT or SLP, focused on the patient’s plan of care and individual goals

Monitor a patient’s psychosocial condition and identify social and emotional needs
