Key Responsibilities

The social worker coordinates medically related social services to assist residents with their physical, mental, and psychosocial needs. They also oversee discharge planning and provide individual, family, and group support to maximize resident well-being.

Requirements

Candidates must hold a bachelor's or master's degree in Social Work and possess a current valid Maryland Social Work license. Experience in a skilled nursing or sub-acute health care setting is preferred.
