The Medical Social Worker will perform home visits to provide clinical social evaluations and develop care plans in collaboration with a multidisciplinary team. They will also act as an advocate for patients and families while connecting them with necessary community resources.
Candidates must hold a Master's degree in Social Work and be licensed by the VA Board of Social Work Examiners at the LMSW or LISW level. A minimum of 2 years of community-based experience is required, along with strong communication and computer skills.
