Overview
Housing Works Community Healthcare (HWCH) seeks a compassionate, clinically skilled Licensed Master Social Worker (LMSW) to provide integrated behavioral health services within the Hook Up and Bridge (HUB) Program and the East New York Primary Care Clinic.
This hybrid role allocates 50% of time to HUB program activities focused on HIV prevention, harm reduction, and behavioral health integration, and 50% of time to providing outpatient mental health counseling services within the ENY primary care clinic.
The LMSW will support individuals disproportionately impacted by HIV, substance use, housing instability, trauma, and structural barriers to care.
Responsibilities
The LMSW will provide evidence-based brief interventions, mental health screening, crisis support, and counseling using Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT)-informed interventions, trauma-informed care, and harm reduction principles. The position plays a key role in improving engagement in HIV prevention and treatment services, supporting behavioral health stabilization, and strengthening linkage to ongoing mental health care.
HUB Program Responsibilities (50%)
- Provide integrated behavioral health services to HUB clients at East New York and Cylar House.
- Conduct mental health and substance use screenings using validated tools, including: PHQ-2 / PHQ-9 DAST-10 AUDIT-C
- Additional screening tools as clinically indicated
- Complete biopsychosocial and risk assessments to identify behavioral health needs and barriers to HIV prevention or treatment engagement.
- Deliver up to three brief intervention sessions using evidence-based approaches including MI, CBT-informed interventions, solution-focused therapy, and trauma-informed care.
- Provide crisis intervention, de-escalation, and safety planning when clinically indicated.
- Collaborate closely with Navigators, Outreach Workers, and Medical Providers to support: HIV testing engagement PrEP initiation and retention PEP access Immediate ART linkage
- Harm reduction service utilization
- Support warm handoffs to higher levels of behavioral health care, psychiatry, substance use treatment, or supportive services within 30 days.
- Participate in case conferences and interdisciplinary care planning.
- Support program quality improvement activities and help identify behavioral health barriers affecting HUB performance metrics.
Primary Care Behavioral Health Responsibilities (50%)
- Provide individual psychotherapy and mental health counseling to ENY primary care patients.
- Maintain a billable caseload of patients receiving ongoing outpatient behavioral health treatment.
- Develop treatment plans with measurable goals and interventions.
- Conduct routine reassessments and update treatment plans in accordance with regulatory and payer requirements.
- Coordinate care with primary care providers, psychiatrists, care coordinators, and case managers.
- Support management of depression, anxiety, trauma, adjustment disorders, substance use, and other behavioral health concerns commonly seen in primary care settings.
- Deliver short-term and long-term counseling interventions based on client needs.
- Facilitate referrals for psychiatric evaluation, medication management, and specialty mental health care as needed.
- Documentation and Compliance
- Complete timely, accurate, and compliant clinical documentation in eClinicalWorks (eCW).
- Maintain documentation in accordance with: HIPAA NYS Mental Hygiene standards Article 28 / Article 31 regulations Housing Works policies and procedures
- Ensure all screenings, assessments, treatment plans, progress notes, and discharge summaries are completed within required timeframes.
- Participate in chart audits, utilization reviews, and quality assurance activities.
- Collaboration and Team Participation
- Participate in weekly clinical supervision and case review with licensed behavioral health leadership.
- Attend interdisciplinary team meetings, QI meetings, and staff trainings.
- Collaborate with Prevention, Harm Reduction, Primary Care, and Behavioral Health teams to ensure integrated care delivery.
- Support a low-threshold, client-centered care model emphasizing dignity, autonomy, and engagement.
Requirements
- College degree required; Advanced degree (MSW, MPH, MPA, or related filed) preferred Master’s degree in Social Work (MSW) from an accredited program.
- Active New York State Licensed Master Social Worker (LMSW) required.
- Minimum 2 years of clinical experience in behavioral health, medical social work, HIV care, substance use treatment, or community health preferred.
- Experience working with marginalized populations, including: LGBTQ+ communities
- People living with or vulnerable to HIV
- People who use drugs Individuals experiencing homelessness or housing instability
- Experience providing trauma-informed counseling and crisis intervention.
- Strong understanding of harm reduction principles.
- Experience with electronic health records (eCW preferred).
