Job Description
Bay Cove Human Services’ mission is to partner with people to overcome challenges and realize personal potential. Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts.
Job Summary
As a per-diem member of the Cape Cod Community Crisis Stabilization (multidisciplinary teams, the Medical Prescriber provides timely psychiatric evaluation, medication assessment and management, medication reconciliation, psychoeducation, and clinical consultation.
The prescriber supports continuity between crisis stabilization and ongoing community-based care through consultation, shared treatment planning, discharge coordination, and warm handoffs.
Services may be delivered in person or through approved telehealth workflows based on clinical need, program expectations, and applicable requirements. Employment classification will be confirmed with Human Resources.
Essential Functions of Position
Provide psychiatric assessment, medication evaluation and management, and clinical consultation for individuals receiving Cape Cod CCS and/or CBHC services.
Collaborate with CCS and CBHC clinicians, nurses, the Nurse Manager, the Medical Director, and community providers to support safe prescribing, continuity of care, and coordinated treatment.
Support CCS admission, stabilization, utilization-review, and discharge workflows, including medication reconciliation, prescriptions needed for transition, and timely communication with follow-up providers.
Provide in-person and/or approved telehealth coverage in accordance with the confirmed per-diem schedule, clinical needs, licensure requirements, and Bay Cove policy.
Respond to urgent medication and psychiatric consultation needs during assigned coverage hours and follow established escalation and on-call procedures.
Provide psycho-pharmacotherapy prescribing with basic evidence-based medical monitoring and medication reconciliation, referring to other medical services when appropriate
Maintain complete and timely clinical records in eHana within Bay Cove, CCS, CBHC, payer, and regulatory timeframes, including psychiatric evaluations, medication orders, medication reconciliation, progress notes, treatment-plan contributions, consultations, and discharge documentation when applicable.
Contribute to comprehensive assessment, risk review, stabilization planning, treatment planning, and discharge planning for individuals receiving CCS and CBHC services; provide case consultation as needed.
Coordinate with the primary care providers and any specialty medical provider to ensure a team-based approach to jointly address the individual’s behavioral health and medical needs
Participate in CCS/CBHC clinical case reviews, case conferences, and required team meetings when they occur during assigned hours or when participation is otherwise arranged.
Perform recommended metabolic and medication monitoring, review relevant laboratory results, order routine blood levels when within scope and clinically indicated, and administer injectable psychiatric medications when required and operationally supported.
Completes or assists in the completion of relevant client-related documentations like disability determination applications, guardianship affidavits, etc.
Complete all training, credentialing, system-access, supervision, and compliance requirements established by Bay Cove and the Cape Cod CCS/CBHC.
Participate in CCS/CBHC quality-improvement, access, safety, medication-management, and continuity-of-care initiatives as appropriate to the per-diem role.
Provide per-diem coverage for Cape Cod CCS and/or CBHC psychiatric prescriber needs according to the agreed schedule and scope.
Schedule and minimum availability: 4 hour shifts, weekends (Saturday & Sunday 9am-1pm) holidays, per diem/part-time. No on-call responsibilities.
Regular, required on-site availability.
Requirements for the Position
Master's or doctoral degree in nursing and completion of an accredited psychiatric nurse practitioner program required.
Current, unrestricted Massachusetts registered nurse and advanced practice registered nurse authorization appropriate to the role; maintain all licenses and credentials required to provide assigned in-person and telehealth services.
Current national certification as a Psychiatric-Mental Health Nurse Practitioner through ANCC or another credential accepted for the position.
Maintain all Massachusetts controlled-substance and federal DEA registrations, prescriptive authority, credentialing, and collaborative or supervisory arrangements required for assigned duties.
