Key Responsibilities
Assess hospice patients to identify psychosocial, grief, and community needs while providing counseling and crisis intervention. Facilitate community resources and ensure compliance with Medicare regulations and clinical documentation standards.
Requirements
Requires a Bachelor's or Master's degree in social work and a current state license. Candidates must have at least one year of clinical experience and a valid driver's license with reliable transportation.
