  • Vocational & Life Skills Support: Conduct task analyses and job-site strategies to help residents pursue vocational or avocational goals.

Interdisciplinary Collaboration & Education

  • Team Integration: Collaborate with the Rehab Case Manager (RCM), and interdisciplinary team members to establish goals, coordinate treatment plans, and support discharge planning.
  • Staff & Family Training: Educate caregivers, family members, payers, and direct care staff on compensatory strategies and care plan execution.
  • In-Service Training: Plan and present evidence-based in-service sessions to staff and stakeholders as needed.

Compliance & Documentation

  • Regulatory Compliance: Maintain service delivery standards in alignment with the ASHA Code of Ethics, State Practice Acts, Division of Developmental Disabilities (DDD), CARF standards, HIPAA regulations, and internal company policy.
  • Timely Reporting: Complete initial assessments, monthly progress reports, daily session notes, and discharge summaries within mandated timeframes.
