About the Role

Join Everent Health & Affiliates as a PACE Nurse Practitioner – PRN, Flexible in United States. Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age.

Key Responsibilities

  • at the discretion of the Medical Director and/or Executive Director.

Requirements

  • Licensure (or eligibility for licensure) for advanced nursing practice in KY, including DEA and controlled substance permits, required.
  • Minimum of one year experience in full time advance nursing practice required.
  • Knowledge of physical, mental, and social needs of frail older adults.
  • Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.

Work Schedule

  • Per Diem position.
  • Flexible scheduling available.
