The PACE Nurse Practitioner assesses, initiates, and coordinates direct care for elderly participants within the clinic, home, or facility settings. They work closely with the interdisciplinary team to ensure comprehensive medical support and participate in community outreach and leadership activities.
Candidates must hold a valid advanced nursing practice license in Kentucky, including DEA and controlled substance permits. A minimum of one year of experience in advanced nursing practice and experience working with a frail or elderly population are required.
