Job Description
Position
The Advanced Practice Provider (APP) provides high-quality, patient-centered primary care to Wayne HealthCare patients. The APP assesses, diagnoses, treats, educates, and manages patients within their education, training, certification, licensure, and approved scope of practice. This position is open to qualified Nurse Practitioners (NPs) and Physician Assistants (PAs), including qualified new graduates.
Requirements
- Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.
- Current, unrestricted Ohio NP/APRN or Physician Assistant license.
- Current national board certification appropriate to specialty.
- Ohio Certificate to Prescribe, as applicable.
- Current BLS/CPR certification.
- Ability to obtain and maintain required hospital, payer, Medicare, Medicaid, DEA, and Wayne HealthCare credentialing.
- Primary care, family practice, internal medicine, urgent care, or comparable experience preferred.
- One year of NP/PA experience preferred, but not required; qualified new graduates may be considered.
Responsibilities
- Obtain patient histories and perform comprehensive or focused examinations.
- Assess, diagnose, and manage acute, chronic, and preventive health conditions.
- Develop individualized treatment plans and prescribe medications within applicable laws and scope of practice.
- Order, perform, and interpret laboratory and diagnostic testing.
- Monitor treatment effectiveness, medication response, and patient adherence.
- Provide preventive care, wellness counseling, and patient education.
- Provide telephone consultation and clinical triage as assigned.
- Communicate diagnoses, test results, treatment plans, and follow-up recommendations.
- Identify conditions requiring physician consultation, specialty referral, or higher levels of care.
- Coordinate care with physicians, specialists, hospitals, and other healthcare professionals.
- Maintain accurate, timely medical records and documentation in the electronic medical record.
- Comply with applicable laws, regulations, payer requirements, HIPAA, and Wayne HealthCare policies.
- Participate in quality improvement, patient safety, chart review, and performance improvement activities.
