Key Responsibilities

• Evaluation and treatment planning: 30%.

• CSB: 30%.

• Initiated upon admission and across the continuum of care discharge planning is provided in an effective and efficient manner according to policy and DBHDS discharge protocols.

• Discharge needs are clearly documented and plans for patients recommended for discharge are updated in weekly notes and ready for discharge form in the EHR.

Requirements

• Bachelor's degree in Social Work (BSW) from an accredited college or university.

• Knowledge of population specific factors in assessment and treatment. Knowledge of theories of human behavior and development. Skills and ability in providing individual, family and group counseling. Knowledge of community resources.

• Advanced interviewing skills. Knowledge of Social Work Code of Ethics. Ability to make independent work decisions. Ability to independently maintain clinical records on assigned cases in accordance with hospital policies and governing body regulations.
