- What You’ll Do
- Deliver Patient-Centered Care
- Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
- Develop and implement individualized care plans in collaboration with physicians and the care team
- Provide hands-on therapy to improve mobility, strength, balance, and independence
- Educate patients and caregivers on treatment plans, exercises, and safety strategies
- Drive Outcomes & Recovery
- Monitor patient progress and adjust treatment plans as needed
- Support safe transitions of care and contribute to discharge planning
- Identify additional patient needs and collaborate with interdisciplinary teams
- Collaborate & Communicate
- Partner closely with nurses, therapists, and physicians to ensure coordinated care
- Document patient care and progress accurately and timely within the EMR
- Participate in case conferences and team meetings
- Lead & Support
- Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
- Provide guidance and education to caregivers and support staff
What You Bring
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Minimum 1–2 years of experience (home health experience preferred)
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance

Key Responsibilities
The Physical Therapist will evaluate patients, develop individualized care plans, and provide hands-on therapy to improve mobility and independence in a home health setting. They will also collaborate with interdisciplinary teams, document progress in the EMR, and supervise Physical Therapist Assistants.
Requirements
Candidates must hold a degree from an accredited Physical Therapy program and possess a current state PT license. A minimum of 1–2 years of experience, current CPR certification, and a valid driver’s license are required.
