Behavioral Health Services Coordinator

Join Central City Concern as a Behavioral Health Services Coordinator in Portland, Oregon.

Key Responsibilities

• Work collaboratively with primary care teams, pharmacy and other clinic staff as well as other Central City Concern programs to continuously enhance integrated behavioral health services in primary care.

• Organize and facilitate clinical and operational team meetings.

• Provide client services including outreach calls, availability for warm handoffs and ongoing coordination.

• Coordinate implementation of workflows for specific projects related to behavioral health in primary care.

Requirements

• HSD or GED required.

• Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities as described above with at least two years of experience working in a clinical setting.

• BA/BS in behavioral health/social services or equivalent health care experience preferred.

• Must have an understanding of the causes of homelessness, the medical and psychosocial context of homelessness, and the systems and processes used to help clients secure and maintain health and self-sufficiency.

Benefits & Compensation

• Pay rate: $23.86 - $31.67/hr.

• 20 days PTO.
