About the Role
Join Home Health & Hospice Care as a Occupational Therapist – Home Care – Nashua, NH in United States. Are you an Occupational Therapist looking for a full time 40 hrs. a week position?
Key Responsibilities
- Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain.
- Evaluates results of occupational therapy by evaluating by observing, noting, and evaluating patient’s progress; recommends and implements adjustments as needed.
- Determines occupational therapy treatment plans in consultation with physicians and homecare team.
- Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs; recommending and providing assistive equipment.
