Key Responsibilities
- Conducts and evaluates medication histories, assesses compliance, and suggests modifications for optimal drug product selection in order to achieve desired outcomes; instructs patients on correct use of prescribed medications.
- Instructs patients in correct use of prescribed medications and identify any barriers to patient compliance with drug therapy.
- Individualizes medication regimens using sound principles and understanding of drug absorption, distribution, metabolism, and elimination to establish and improve to optimum patient benefit.
- Effectively verifies patient medication orders for appropriateness and accuracy. Provides medication reconciliation and evaluates medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information.
Requirements
- Clinical—demonstrates a working knowledge of medical theory and practice to evaluate and provide patient care; skill in taking medical history to assess medical condition and interpret findings; maintains knowledge of, and acts in accordance with, current NHC policies and procedures.
- Problem solving – identifies and resolves problems in a timely manner and gathers and analyzes information skillfully.
- Oral communication—speaks clearly and persuasively in positive or negative situations, demonstrates group presentation skills.
- Written communication—edits work for spelling and grammar, presents numerical data effectively and is able to read and interpret written information.
