Job Description
The Nurse Practitioner functions under the clinical supervision of licensed physicians, provides high quality wound care to patients within Skilled Nursing Facilities, Assisted Living, Independent Living, Memory Care, Inpatient Rehabilitation Hospital, Inpatient Psychiatric Hospital settings.
ESSENTIAL DUTIES and RESPONSIBILITIES
- Evaluates and manages patients for wound care and ensures delivery of care meets current standards of practice.
- Ensures education is provided to the facility staff regarding patient's wound(s).
- Address both acute and chronic primary care complaints, and able to ascertain medical urgency.
- Apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
- Consult specialty services as appropriate.
- Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom.
- Accept feedback and direction from the supervising physician.
- Create and implement a medical plan of care.
- Prescribe medications as appropriate.
- Perform timely documentation and billing.

Minimum Qualifications
- Preferred one year experience in Internal Medicine and Wound Care.
- Excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
- Problem-solves and uses resources well independently and as part of a team.
- Willingness to work in a fast-moving, high-growth work setting.
- Knowledge of Electronic Health Record (EHR), e-prescribing software, scheduling software, and telemedicine software.
Education
- Master of Science- Nursing
- Doctorate of Nurse Practice
License/Certification
- Valid Wisconsin Registered Nurse License
- Valid Wisconsin Advanced Practice Nurse Practitioner License
- Valid Wisconsin Physician Assistant License
- Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
- National Commission of Certification of Physician Assistants (NCCPA)
- Basic Life Support for the Healthcare Provider (BLS)
- Wound Care Board Certification (WOCN) or within 6 months of hire
PHYSICAL: While performing the duties of this job, the employee is regularly required to talk, hear and listen. The employee frequently is required to stand, walk and sit. The employee must occasionally lift and/or move up to 50 pounds. Position may require exertion up to 50 pounds of force occasionally and/or up to 10 pounds of force frequently or constantly lift, carry, push, pull or otherwise move objects.
Working Environment
Skilled Nursing, Memory Care, Assisted Living, Independent Living, and Inpatient Rehab Hospital, Inpatient Psychiatric Hospital environments.
