Physical Therapist (PT) - Home Health

Serving patients across Pueblo and surrounding communities | [Full-Time]

Compensation: $130,000- $150,000 annual salary

Schedule: Full-time, Monday through Friday

As a Home Health PT, you will help patients rebuild strength, mobility, balance, and confidence following illness, injury, surgery, or a change in condition. You will combine clinical expertise with autonomy while helping patients work toward the goals that matter in the place they call home.

What You'll Do

As a Home Health PT, you will deliver personalized therapy while helping patients safely regain function and independence:

  • Evaluate mobility, strength, balance, pain, and functional limitations in the home environment
  • Develop and update individualized therapy plans aligned with physician orders and patient goals
  • Provide therapeutic exercise, hands-on interventions, gait training, and mobility instruction
  • Recommend and train patients on appropriate assistive devices and safety strategies
  • Educate patients and caregivers on exercises, fall prevention, and safe techniques
  • Track progress, adjust interventions, and maintain timely, accurate documentation
  • Collaborate with nursing, occupational therapy, speech therapy, and other care team members

What You Bring

  • Graduate of an accredited physical therapy program with an active Colorado PT license
  • Minimum of one (1) of physical therapy experience
  • Home health experience preferred
  • Current CPR certification
  • Strong clinical judgment, communication, organization, and documentation skills
