ABOUT THE ROLE The Community-Based Therapist plays a key role in supporting youth as they transition from institutional care into family or family-like settings.

Key Responsibilities

  • Meet regularly with youth and caregivers in community settings to advance treatment goals using culturally responsive, evidence
  • informed practices
  • Coordinate care and service planning with multidisciplinary teams to support safe, stable transitions for youth and families
  • Collaborate with caregivers, agency staff, schools, county partners, and other providers to develop and implement individualized care plans, behavioral supports, and interventions

Requirements

  • Participate in crisis response and intervention services, including on
  • call rotation when applicable
  • Comply with all mandated reporting requirements and applicable Medicaid/Medi
  • Cal standards

  • Salary: $73K - $87K
  • Signing bonus: $2,000.
