The RN Case Manager will rotate through various territories to manage home health caseloads, perform clinical assessments, and develop individualized plans of care. They are responsible for coordinating with interdisciplinary teams and ensuring patients receive timely, high-quality care in their homes.
Candidates must hold an active Minnesota Registered Nurse (RN) license and possess strong clinical assessment skills. Previous experience in home health and familiarity with OASIS documentation are strongly preferred.
