Requirements
- MD/DO from an accredited educational institution
- BC/BE in Interventional Pain, PM&R, Internal Medicine, Orthopedic Surgery, or Neurology
- Unrestricted license to practice medicine in the state work will be performed
- Favorable malpractice history
