Prava is looking for an entrepreneurial Physical Therapist Practice Owner to build a thriving therapy practice within one of our premier senior living partners in Northern California.
This is not a traditional outpatient clinic.
There is no waiting room full of patients. No revolving door. No treating someone for 45 minutes and sending them back into the community without knowing what happens next.
You are embedded in the resident experience.
You build relationships with residents, families and the senior living team. You identify opportunities for therapy. You develop referral pathways. You lead clinicians. And you create a clinical environment where therapy supports what residents actually want to keep doing—moving, participating, connecting and living independently.

- As a Prava Practice Owner, you have the autonomy to lead your clinical practice while Prava provides the infrastructure behind you:
- EHR and practice management technology
- AI-enabled clinical documentation

Your Role
As the Practice Owner, You Will
Build the Practice
- Establish and grow the physical therapy program within the community.
- Develop strong relationships with residents, families and community leadership.
- Build referral pathways and create awareness of the clinical services available.
- Identify opportunities to expand services as resident needs evolve.
Lead Clinically
- Evaluate and treat residents across the continuum of senior living.
- Establish a high standard of clinical reasoning, documentation and resident-centered care.
- Mentor and develop additional clinicians as the practice grows.
- Create a culture where quality matters as much as volume.
Connect Therapy to Lifestyle
- Partner with the community's wellness and fitness teams.
- Look beyond the traditional therapy episode and think about mobility, participation and long-term function.
- Help residents remain engaged in the activities, relationships and routines that matter to them.
- Bring clinical expertise into the broader senior living environment.
Operate Like an Owner
- Understand your practice's clinical and financial performance.
- Monitor volume, utilization, referrals and growth.
- Participate in Prava's Practice Owner Operating Rhythm.
