Key Responsibilities

Conduct social history assessment of the patient through observation and patient/family interview, obtaining.

Coordinate and plan programs and activities to meet patient’s social and emotional needs.

Manage a caseload and assume responsibility for patient chart documentation.

Requirements

Candidates must possess a Master's degree in Social Work, Psychology, Counseling, or a Human Services field, along with one or more years of experience working with the behavioral health patient population. Required credentials include a current Ohio Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Social Worker (LSW), plus current CPR and CPI certifications.
