Physical Therapist (PT) - Legacy Nursing and Rehabilitation of Lafourche
LP Therapy
LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
Responsibilities: review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.
Qualifications: Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills.
Work Environment: LTC facility; physical demands include lifting and mobility assistance.
Key Responsibilities
The Physical Therapist will evaluate patients, develop treatment plans, and administer therapeutic exercises to restore mobility and function. They are also responsible for collaborating with care teams and maintaining accurate Medicare documentation.
Requirements
Candidates must hold a Bachelor's degree in Physical Therapy and possess a current state PT license. Experience in a Long-Term Care setting and knowledge of Medicare guidelines are preferred.
