NURSE PRACTITIONER NP - HOSPICE & PALLIATIVE CARE SERVICES - PRN

Responsibilities
- Proactive, collaborative team member in a hospice care environment
- Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
- Commitment to caring for patients and partners
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a hospice setting

Icensure, Certification, and Educational Requirements
Current, active, and unrestricted Registered Nurse (RN) and Nurse Practioner (NP) licensure in state of practice
Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care

Requirements
Candidates must hold an active, unrestricted Registered Nurse and Nurse Practitioner license in the state of practice. Board certification by AANP or ANCC in adult, family, geriatric, or adult-gerontology primary care is required.
