Salary Range: The salary range for this position is $89.00 - $104.00 per visit.

What you will need to be successful

* You have a Bachelor of Social Work or a Master of Social Work
* You are a registered member in good standing with the Ontario College of
Social Workers and Social Service Workers
* 1 to 2 years of community experience is an asset
* You are passionate, empathetic, use good judgement and apply critical
thinking when making client decisions
* You are known for your excellent communication skills and ability to work in
collaboration within an interdisciplinary team as well as work independently
* You have ability to travel within the Hawkesbury community as part of the CBI
Health Home Health Services team
* Your bilingual language skills in English and French are a MUST have
