Key Responsibilities

• Provide one-on-one OT services in patients’ homes, focusing on safety, function, and independence.

• Complete comprehensive assessments and develop individualized treatment plans.

• Train patients and families on adaptive equipment, energy conservation, and home safety strategies.

• Collaborate with an interdisciplinary team (nursing, PT, social work, aides, etc.) to coordinate care.

Requirements

• Current Occupational Therapist (OT) license in the state of practice.

• Graduation from an accredited Occupational Therapy program.

• Previous home health, SNF, or rehab experience preferred, but strong new grads will be considered.

• Valid driver’s license, reliable transportation, and appropriate auto insurance.
