Job Description
About the Role
At Corus Health, we are known for warm, steady, and mission-anchored care. As a Medical Social Worker on our hospice team, you'll walk alongside patients and families during some of life's most significant moments. You'll help them understand their options, find support, and feel less alone. This role is for a social worker who brings calm, clear thinking, and real compassion into the home.
Key Responsibilities
- Provide social work services to hospice patients and families under a physician's plan of care
- Help the interdisciplinary team understand the social, emotional, and environmental factors shaping each patient's care
- Connect patients and families with community resources, financial support, and social services
- Help develop and carry out individualized care plans
- Contribute to planning that supports safe, smooth transitions
- Partner with community organizations and make referrals when they will help
- Join patient care conferences and consult with your teammates
Document your assessments and interventions clearly and on time, in line with Medicare CoPs, state regulations, and professional social work standards
Requirements
- What We're Looking For
- A Master's degree in Social Work from a CSWE-accredited program
- LMSW or LCSW license as required by the state of New Mexico
- At least one year of social work experience in a healthcare setting
- Knowledge of community resources and psychosocial care planning
- Strong communication, assessment, and advocacy skills
- Comfort with EMR systems and basic computer skills
- A valid driver's license, reliable transportation, and auto insurance for home visits
- The independence and initiative to do this work well in the field, with a team behind you
