Key Responsibilities
Provide coordinated physical therapy care to clients of all ages by planning, implementing, and evaluating individualized treatment plans. Perform clinical assessments, document progress notes, and instruct patients and families on transfer techniques and equipment usage.
Requirements
Requires a Bachelor of Science in Physical Therapy and at least two years of clinical experience, with home health experience preferred. Must hold a current state professional license, CPR certification, and a valid driver's license with reliable transportation.
