Key Responsibilities
The Nurse Practitioner will conduct comprehensive health evaluations and manage comorbid conditions for patients with complex kidney conditions. They will lead clinical teams to coordinate care, reduce hospitalizations, and improve patient outcomes through effective disease management.
Requirements
Candidates must have a minimum of 2 years of experience as a Nurse Practitioner and hold a current, unrestricted NP license. Proficiency in managing complex comorbidities and intermediate computer skills are required, along with a valid driver's license.
