Key Responsibilities
The Physical Therapist will perform evaluations, develop and re-assess treatment plans, and provide rehabilitation services in home and community settings. They will also collaborate with the treatment team, physicians, and families to ensure coordinated care and improved patient outcomes.
Requirements
