Key Responsibilities

The provider will perform clinical evaluations, determine medical eligibility, and manage patient care transitions from the hospital to the home setting. They will collaborate with interdisciplinary teams to ensure high-quality, inpatient-level care while maintaining program standards and patient safety.

Requirements

Candidates must hold a Master's degree from an accredited nursing or Physician Assistant program and possess current Illinois licensure. A minimum of 5 years of acute care experience in fields such as Hospital Medicine, Emergency Medicine, or Critical Care is preferred.
