Key Responsibilities

The Case Manager facilitates the assessment, planning, and advocacy for patient health needs to ensure quality and cost-effective outcomes. They also coordinate the continuum of care and serve as a liaison between the interdisciplinary team, patients, and families.

Requirements

Candidates must have a degree from an accredited school of social work, nursing, or a related field. Five years of experience in the healthcare field and previous case management experience are preferred.
