Key Responsibilities
- As a Family Nurse Practitioner (FNP) with HealthTrac @ Health Solutions, you will play a central role in delivering integrated, patient-centered care and APM performance metrics.
- Order, perform, and interpret diagnostic tests (e. g., blood work, imaging, EKGs, urinalysis) to support accurate diagnoses.
- Develop and adjust individualized treatment plans, prescribing medications, therapies, and preventive interventions as appropriate.

- Adhere to confidentiality standards, medical ethics, and Colorado statutes related to collaborative practice and prescriptive authority.
- Professional Development Stay current with medical research and guidelines through continuing education and integration of evidence-based practices.

- Physical Requirements: Mobility & Stamina: Ability to stand and walk for extended periods (typically 8–12 hours), frequently moving between exam rooms and workstations.

Licensure & Certifications
