Key Responsibilities
Provide one-on-one physical therapy to older adults in their homes or senior living communities using the Geriatric House Calls model. Collaborate with an interdisciplinary team to develop individualized care plans and document progress within the EMR system.
Requirements
Must hold a valid Physical Therapy license or be eligible to obtain one in the state of practice. Requires a degree from an accredited Physical Therapy program and the ability to perform physical tasks such as lifting and patient transfers.
