Job Description

  • Advanced Heart Failure Cardiologist — Program Leadership
  • Advanced Heart Failure Cardiologist / Heart Failure Program Director

Lead and shape a comprehensive community heart failure program with a high-volume shock team, mechanical circulatory support (MCS/ECMO), and collaborative pathways with regional academic transplant centers.

Legacy Medical Group (LMG), a physician-led health system in the greater Portland, Oregon metropolitan area, is seeking a dynamic, forward-thinking BC/BE Advanced Heart Failure and Transplant Cardiologist to lead and expand our community-based Advanced Heart Failure Program. This physician will spearhead programmatic development, clinical operations, and cardiogenic shock care across our multi-hospital network, working collaboratively with interventional cardiology, cardiothoracic surgery, critical care, and regional academic partners.

Responsibilities

Role Overview & Program Leadership

Programmatic Leadership: Serve as the clinical and operational leader for heart failure care across the system, establishing standardized GDMT protocols, transition-of-care pathways, and outpatient GDMT optimization clinics.

Cardiogenic Shock & Critical Care: Provide leadership for an active, multidisciplinary 24/7 Shock Team, utilizing standardized invasive hemodynamic assessment and rapid escalation protocols.

Mechanical Circulatory Support & ECMO: Direct clinical management of temporary and durable MCS, including percutaneous ventricular assist devices (Impella CP/5.5, ProtekDuo, intra-aortic balloon pumps), veno-arterial (VA) ECMO, and acute-on-chronic decompensated heart failure.

Device Therapy & Remote Monitoring: Oversee ambulatory hemodynamic monitoring (CardioMEMS), Barostim and Cardiac Contractility Modulation (CCM).

Academic Transplant Center Collaboration: Foster direct clinical relationships and bridge-to-transplant/bridge-to-durable LVAD referral algorithms with local transplant programs, with potential opportunities for shared clinical conferences, joint committees, and cross-institutional collaboration.

Clinical Model & Resources

Dedicated Inpatient & Outpatient Structure: Protected time divided between advanced heart failure inpatient consults/ICU rounds and dedicated outpatient heart failure clinic, supported by advanced practice providers (APPs), dedicated nurse navigators, and clinical pharmacy specialists.

Multidisciplinary Shock Platform: Seamless co-management alongside critical care intensivists, experienced interventionalists (1,350+ PCIs system-wide), and cardiothoracic surgeons.

Advanced Imaging & Diagnostics: Full access to high-volume echocardiography, cardiac MRI, cardiac CT, and comprehensive invasive hemodynamic catheterization.

Qualifications

Education: MD or DO from an accredited medical school.

Training: Completed ACGME-accredited Internal Medicine Residency, Cardiovascular Disease Fellowship, and an accredited Advanced Heart Failure & Transplant Cardiology Fellowship.

Board Certification: Board Certified/Eligible in Cardiovascular Disease and Advanced Heart Failure & Transplant Cardiology.

Clinical Expertise: Documented procedural competence in right heart catheterization, endomyocardial biopsy (preferred), and temporary MCS/ECMO management.

Licensure

  • Visa Status: Unable to sponsor H-1B or J-1 visa waivers at this time.
