Key Responsibilities
Provide physical therapy services to patients in their home environment under the direction of a qualified physical therapist. Participate in interdisciplinary assessments and maintain accurate, timely documentation of patient progress and treatment interventions.
Requirements
Must hold an active Physical Therapist Assistant license in the State of Minnesota and have at least two years of experience. Requires a valid driver's license, reliable transportation, and current CPR/BLS certification.
