Pharmacist - PRN

> The PRN Pharmacist is responsible for providing safe, accurate, and clinically appropriate pharmacy services to patients receiving infusion and specialty therapy. This position works collaboratively with nurses, providers, and other members of the healthcare team to ensure optimal medication therapy, appropriate monitoring, and safe transitions from hospital or clinic to home.

Key Responsibilities

Potential Responsibilities Include

• Review and verify home infusion medication orders for accuracy, appropriateness, and safety
• Assess patient-specific medication regimens, including dose, route, frequency, duration, renal/hepatic function, allergies, and drug interactions
• Monitor and manage pharmacokinetic dosing, particularly vancomycin and aminoglycosides
• Review laboratory results and make appropriate recommendations regarding medication therapy
• Collaborate with physicians, advanced practice providers, nurses, and other healthcare professionals
• Provide clinical recommendations related to antimicrobial therapy and other infusion medications
• Ensure compliance with applicable pharmacy, state, federal, and accreditation requirements
• Provide education to patients, caregivers, and healthcare professionals when appropriate
• Review and oversee the preparation of compounded sterile preparations in accordance with established procedures
• Maintain accurate and timely pharmacy documentation
• Communicate with home infusion nurses and doctors offices regarding medication changes, laboratory monitoring, adverse effects, and patient-specific concerns
• Assist with after-hours or urgent clinical pharmacy needs as part of the PRN team (Taking Call/Being On-Call)
• Maintain current knowledge of home infusion therapies, antimicrobial stewardship, and relevant clinical guidelines

Qualifications

Required

• Doctor of Pharmacy (PharmD) or equivalent pharmacy degree
• Active pharmacist license in the applicable state
• Strong clinical assessment and medication-management skills
• Excellent written and verbal communication skills
• Strong attention to detail and commitment to medication safety

Preferred

• Previous home infusion, hospital, ambulatory care, specialty pharmacy, or sterile compounding experience
• PGY-1 residency or equivalent clinical experience
• Experience with pharmacokinetics and therapeutic drug monitoring
• Experience with antimicrobial therapy
• Experience with USP <797> sterile compounding requirements
• Experience with electronic medical records/pharmacy information systems
