  • Develop, implement, and update individualized treatment plans in collaboration with clients and the interdisciplinary treatment team.
  • Provide ongoing medical and psychiatric follow-up, including assessment of changes in mental status, physical health, withdrawal risk, medication response, and overall clinical stability.
  • Support evidence-based treatment for substance use disorders, including medication-assisted treatment (MAT) / medications for opioid use disorder (MOUD) when clinically appropriate and within credentialed scope.
  • Collaborate with therapists, nurses, physicians, case managers, peer support professionals, behavioral health staff, and program leadership to coordinate care.
  • Communicate clinical findings, treatment recommendations, medication changes, and safety concerns clearly and promptly to the appropriate members of the treatment team.
  • Coordinate referrals, laboratory work, drug screening, specialty care, and transitions of care as clinically indicated.
  • Support safe medication administration, prescription verification, medication reconciliation, and appropriate medication-management processes.
  • Document assessments, diagnoses, treatment decisions, medication changes, client interactions, and care coordination accurately and promptly in the electronic medical record (EMR).
  • Participate in treatment planning, clinical consultation, team meetings, family collaboration, and other interdisciplinary activities as appropriate.
  • Educate clients regarding diagnoses, medications, treatment recommendations, recovery supports, and available services to promote informed participation in care.
  • Recognize and respond appropriately to urgent medical or psychiatric concerns and coordinate escalation of care when needed.
  • Contribute to program development, quality improvement, clinical workflows, staff education, and initiatives that strengthen client care and organizational effectiveness.
  • Maintain client confidentiality, professional boundaries, and respectful, trauma-informed care consistent with WisHope policies and applicable professional standards.
  • Travel between WisHope program locations as needed to support client care and organizational needs.
