Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.
Key Responsibilities
- Assess the psychosocial needs of the patient and family members.
- Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues.
- Contribute to and execute the Plan of Care with the family/patient.
- Record daily documentation of visits and contact in accordance with Policy.
Requirements
- Bachelor's of Social Work with 1 year of applicable experience required; Master's degree preferred
- Texas State license required
- Reliable transportation and a valid driver's license.
- Hospice or healthcare experience preferred.
