Join Community Healthcare of Texas as a Social Worker – Hospice & Palliative Care in Texas. Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996. Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.

Key Responsibilities

  • Assess the psychosocial needs of the patient and family members.
  • Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues.
  • Contribute to and execute the Plan of Care with the family/patient.
  • Record daily documentation of visits and contact in accordance with Policy.

Requirements

  • Bachelor's of Social Work with 1 year of applicable experience required; Master's degree preferred
  • Texas State license required
  • Reliable transportation and a valid driver's license.
  • Hospice or healthcare experience preferred.
