Children's Social Worker, Part-Time

Key Responsibilities

The Social Worker assists patients with complex psychosocial needs by determining eligibility for social programs and coordinating safe discharge plans. They work alongside RN Case Managers to identify optimal resources for patients and their caregivers throughout the care continuum.

Requirements

Candidates must hold a Bachelor's degree in Social Work from an accredited program. Experience in hospital social work and knowledge of managed care practices are preferred.
